Adult Dose

1 Chronic myeloid leukemia Oral 60 (or 1.8 mg/m2) micrograms/kg Tablet A usual maximum single daily dose of 4 mg. This (regimen) is continued until the white cell count has fallen to between 15 000 and 25 000 cells/mm3 (typically 12 to 20 weeks). It should be stopped earlier if the platelet count falls below 100 000 cells/mm3. Higher doses may be given if the response after 3 weeks is inadequate but this increases the risk of irreversible damage to the bone marrow and calls for special vigilance. Once an initial remission has been attained treatment is stopped and not resumed until the white cell count returns to 50 000 cells/mm3. If this occurs within 3 months continuous maintenance treatment with a usual dose of 0.5 to 2 mg daily may be given.
2 Polycythaemia vera Oral 4 6 mg Tablet Given daily, continued for 4 to 6 weeks with careful monitoring of blood counts. Further courses are given when relapse occurs; alternatively, maintenance therapy may be given at half the dose required for induction. Doses of 2 to 4 mg daily have been given for essential thrombocythaemia or myelofibrosis.
3 Haematopoietic stem cell transplantation Oral 1 mg/kg Tablet q.i.d. In conditioning regimens for haematopoietic stem cell transplantation busulfan is given in above doses every 6 hrs for 4 days starting 7 days before transplantation; cyclophosphamide is usually given for 2 days, starting 24 hrs after the final dose of busulfan. When given by intravenous infusion, the recommended dose is 800 micrograms/kg ideal body-weight every 6 hrs for 4 days starting 7 days before transplantation.

  ►    Mechanism of Drug Drug Interaction :  Because of their effects on the gastrointestinal mucosa antineoplastics have the potential to interfere with the absorption of drugs given orally. Many antineoplastics are inhibitors of certain isoenzymes of cytochrome P450 and some antineoplastics are also metabolised by these enzymes, and in consequence the possibility of interactions between antineoplastics, or between antineoplastics and other medication, cannot be discounted. Phenytoin increases the metabolism of busulfan.,

  ►  Mechanism of Action :   As a class, the alkylating agents exert their cytotoxic effects via transfer of their alkyl groups to various cellular constituents. Alkylations of DNA within the nucleus probably represent the major interactions that lead to cell death. However, these drugs react chemically with sulfhydryl, amino, hydroxyl, carboxyl, and phosphate groups of other cellular nucleophiles as well.
